In this role, you’ll assess learning needs, create personalized education plans, and deliver both one-on-one and group instruction on all aspects of diabetes self-management. Your expertise will guide patients and families in making informed lifestyle choices, while also supporting staff and serving as a trusted resource for the community.

Annual Salary Range: $76,835.20 - $113,339.20
Qualifications
Education:
-
Requires an Associate of Science in Nursing or Bachelor of Science in Nutrition.

If RN, prefer a Bachelor of Science in Nursing.
Experience:
-
Requires a minimum of three (3) years as an RN or RD.

Prefer five to ten (5-10) years working as a Diabetes Educator.
Certification/Licensure:
-
Must possess a current, valid Arizona RN or RD license.

If RN, must be in good standing with the AZ Board of Nursing.

Requires CDCES (Certified Diabetes Care and Education Specialist) certification.

Requires a BLS card obtained through an approved American Heart Association (AHA) training center within three months of hire.

Ambulatory Clinics: Must have a valid fingerprint clearance card issued by the Arizona Department of Public Safety or submit a completed Affidavit at the time of hire. Must complete the fingerprint clearance card application within seven (7) days of the hire date. Must present actual card within 90 days of applying for a fingerprint clearance card.
Knowledge, Skills, and Abilities:

Requires clinical expertise and advanced understanding of Diabetes Education.

Requires strong leadership, mentoring, communication, and organizational skills.

Requires the ability to read, write, and speak effectively in English.

Requires ability to approach patients with tact and sensitivity.

Bilingual in English and Spanish is strongly preferred.

Must have transportation and be willing to travel to various ambulatory health centers.
