Facility: Valleywise Comprehensive Health Center - Phoenix
Department: FQHC Diabetes Care & Support - Phoenix
Schedule: Regular FT 40 Hours Per Week
Shifts: Days
As a crucial part of our team, the Diabetic Educator, under the general supervision of the CHC Manager, plays a vital role in assessing the learning needs of patients, family members, and staff. They provide individual and group instruction on diabetes self-management topics as needed, evaluate the effectiveness of the educational process, and serve as a valuable resource on diabetes-related topics for patients, staff, and the community.
Must be bilingual (English and Spanish).
Annual Salary Range: $68,307.20 - $100,755.20
Qualifications
Education:
- Requires an Associate of Science in Nursing or Bachelor of Science in Nutrition.
- If RN, prefer a Bachelor of Science in Nursing.
Experience:
- Requires a minimum of three (3) years as an RN or RD.
- Prefer five to ten (5-10) years working as a Diabetic Educator.
Certification/Licensure:
- Must possess a current, valid Arizona RN or RD license.
- If RN must be in good standing with the AZ Board of Nursing.
- Requires CDE certification.
- Requires a BLS card obtained through an approved American Heart Association (AHA) training center within 3 months of hire.
Knowledge, Skills, and Abilities:
- Requires clinical expertise and advanced knowledge of Diabetic Education.
- Requires strong leadership, mentoring, communication, and organizational skills.
- Requires the ability to read, write, and speak effectively in English.
- Requires an ability to approach patients with tact and sensitivity.
- Must have transportation and be willing to travel to various ambulatory health centers.
