Licensed Substance Abuse Specialist - ACT (Assertive Community Treatment) Team
Empowering Recovery. Restoring Lives. Out in the Community, Where It Matters Most.

At Valleywise Health, we go where the need is greatest-and as a Licensed Substance Abuse Specialist on our Assertive Community Treatment (ACT) Team, you’ll bring life-changing support directly to individuals living with serious mental illness and co-occurring substance use disorders.

Under the guidance of the ACT Clinical Coordinator, you will serve as the team’s subject matter expert on substance use, delivering integrated, person-centered care in the community, at members’ homes, and even at their places of employment. This is a role for someone who believes in meeting people where they are-both physically and in their recovery journey.

What You’ll Do:
  • Provide specialized substance use treatment and guidance as part of a multi-disciplinary ACT team
  • Collaborate with team members to develop and implement Individual Service Plans (ISPs) tailored to each member’s goals and needs
  • Offer or coordinate all substance use-related services, including assessment, harm reduction, relapse prevention, and recovery support
  • Deliver care in the field-wherever members live, work, and connect with their community
  • Serve as a consistent and supportive presence, helping members navigate co-occurring challenges and move toward recovery on their terms
  • Participate in the ACT team’s 24/7/365 on-call rotation, ensuring continuous support for members at all times
  • Champion recovery-oriented, trauma-informed practices that emphasize dignity, autonomy, and lasting change
If you’re a licensed clinician with a passion for substance-use recovery and a heart for community-based care, this is your chance to transform lives outside the walls of traditional healthcare.
