Department: BH Specialty Clinic - Mesa

Schedule: Regular FT 40 Hours Per Week

Shifts: Days

Under the direction of the Clinical Coordinator, the Rehabilitation Specialist is responsible for assisting members in understanding their strengths and interests, developing employment goals, and enhancing readiness and commitment to pursue employment. This position is responsible for educating, coaching, training and demonstrating skills required for securing and maintaining employment. This position is responsible for the provision of job development and job coaching services, including maintaining strong relationships with community employers. The Rehabilitation Specialist has lead responsibility for identifying and incorporating vocational goals and services in the Individual Service Plan (ISP), direct service provision and/or coordination of services and monitoring for successful outcomes. Committed to meeting the member where they are and providing services in the community and home as appropriate
 
Hourly Pay Rate: $18.24 - $26.90
 
Qualifications
Education:
  • Must meet one of the following combinations of Education and Experience: 
    • A Bachelor's Degree in a field related to Behavioral Health; OR
    • Any other Bachelor's Degree and one (1) year of full-time behavioral health work experience; OR
    • Any Associates Degree and two (2) years of full-time behavioral health work experience; OR
    • Eighteen (18) credit hours of post-high school education in a field related to behavioral health and two (2) years of full-time behavioral health work experience; OR
    • A high school diploma or GED and four (4) years of full-time behavioral health work experience.
Experience:
  • Requires a minimum of one (1) year of experience working with adults with a serious mental illness that demonstrates an understanding of the required knowledge, skills and abilities.
  • Previous experience working with providing vocational and/or rehabilitation services is preferred and may substitute for required experience.
Certification/Licensure:
  • Requires possession of, and ability to retain a current, valid AZ issued driver's license in good standing, and must carry the AZ legally required minimum insurance coverage on personal vehicle at all times. 
  • Must obtain fingerprint card as defined in Arizona Revised Statute (A.R.S.) 36-425.03 and state licensing rule R9-10-1011(A)(4) within 7 days of employment.  
  • Requires BLS. A BLS card obtained through an approved American Heart Association (AHA) training center is preferred and will be required within 3 months.
Knowledge, Skills and Abilities:
  • Must have in-depth knowledge of SAMHSA's Supported Employment evidence-based model, the Americans with Disabilities Act, the Social Security Administration's Ticket to Work Program and Work Incentives, the Rehabilitation Act and Vocational Rehabilitation program, the Workforce Investment Act, and other policies that impact persons with disabilities in the workplace.
  • Requires the ability to assess needs in order to design and implement rehabilitation programs including personal and vocational counseling, training, and job placement. 
  • Must be able to develop and maintain relationships with community referral sources. 
  • Requires skills in motivational interviewing. 
  • Must be able to work collaboratively with member's family and other professionals. 
  • Requires the ability to have a reliable form of transportation to travel throughout the community with transportation of program participate as needed. 
  • Must have good verbal and interpersonal skills to interact with clients, visitors, and coworkers. 
  • Requires the ability to maintain highly confidential information, including a working knowledge of HIPAA. 
  • Must have the ability to work independently and seek supervision appropriately. 
  • Must be at least 21 years of age; or at least 18 years of age and licensed as a Nurse, Physician Assistant, or Behavioral Health professional.
  • Requires the ability to read, write and speak effectively in English.
  • Pursuant to Arizona Administrative Code R9-10-306 CHAPTER 10 of the Dept. of Health Services, personnel must be at least 21 years of age, or at least 18 years of age and licensed or certified under A.R.S. Title 32, and providing services within the personnel member’s scope of practice.
